What all vaccination records shall I bring to the USCIS medical exam?

If you do the labs ordered by the USCIS civil surgeon within 24 hours of the first appointment, it usually takes a week if no additional findings regarding TB or any other lab come negative. Additional tests like an X-ray might be needed if the TB test comes back positive, which might need another few days.

  2. Complete the top section for pages 2-14 Family Name (Last Name), Given Name (First Name), AA – Number (if any). **This is located at the top of every page. **
  3. Bring completed I-693 forms to your appointment.
  4. Bring your Covid vaccination card, immunization records (if any), insurance card, and photo ID to your appointment. (We cannot accept a digital version on your phone. We need a physical copy to attach to your packet.)
